603 Health provides wellness-oriented access services, including:
- Facilitating access to physician-authorized lab requisitions through Fullscript / Rupa Health, our clinical authorization partner
- Coordinating laboratory testing with independent labs such as Quest Diagnostics
- Delivering requisitions and results
- Providing general educational information to support understanding of lab results
These services help users access deeper wellness insights but do not diagnose, treat, or manage medical conditions.
1.1 What We Are NOT
603 Health is NOT:
- ❌ A medical practice
- ❌ A healthcare provider or telemedicine provider
- ❌ A diagnostic service
- ❌ A treatment or therapy provider
- ❌ A laboratory
- ❌ A seller of laboratory tests
- ❌ A substitute for professional medical care
Using SimpleLabs does not create a doctor–patient relationship.
2. Eligibility and User Obligations
2.1 Age Requirement
You must be 18 years or older to use our services.
2.2 Accurate Information
You agree to provide truthful, complete, and current information during purchase and intake.
Failure to do so may delay or invalidate your requisition.
2.3 Account Security
You are responsible for maintaining the confidentiality of any login credentials.

By using our services, you acknowledge:
- Lab results are for informational and educational purposes only
- Lab results are not medical advice
- We do not interpret results or provide clinical recommendations
- You must consult a licensed healthcare provider for medical decisions
- You will not use SimpleLabs for urgent or emergency situations (call 911)
- You accept full responsibility for how you use the information provided
No content on our website or platform should be construed as medical advice.
4. Lab Services and Process
4.1 Lab Requisitions
- Requisitions are generated by licensed clinicians through Fullscript / Rupa Health
- Requisitions are typically delivered within ~24 hours
- You are responsible for verifying appointment requirements, fasting instructions, or identification required by the lab
4.2 Laboratory Testing
Laboratory testing is performed independently by Quest Diagnostics or other accredited labs.
603 Health does not perform laboratory testing.
Your SimpleLabs purchase includes Quest Diagnostics' standard specimen collection fee, meaning:
- You will not be charged a collection fee at your Quest location
- You may visit any participating Quest office without additional payment
Lab turnaround times (typically 3–5 business days) may vary. Results are delivered electronically
when released by the laboratory.
4.3 State Restrictions
Some states restrict practitioner-mediated or third-party-facilitated lab ordering.
New York, New Jersey, and Rhode Island:
Orders may require additional steps, alternative processing, or may not be available.
We notify users automatically if restrictions apply to their order.
5. Payment, Pricing, and Inclusions
5.1 Payment Terms
- Payment is due at checkout
- Payments are securely processed by Stripe
- All prices are listed in U.S. Dollars (USD)
- Prices may change at any time
5.2 What's Included
Your purchase includes:
- Coordination of physician authorization via Fullscript / Rupa Health
- Your digital lab requisition
- Coverage of Quest Diagnostics' standard specimen collection fee
- Delivery of lab results
- Access to wellness-oriented educational resources
5.3 What's NOT Included
Your purchase does not include:
- Medical interpretation or personalized clinical advice
- Follow-up consultations
- Replacement or repeat tests
- Insurance billing or reimbursement services
- Specialty specimen handling fees if required by a specific test (rare; you will be notified if applicable)
5.4 Taxes
Applicable taxes are calculated at checkout based on your location.
6. Refund Policy
ALL SALES ARE FINAL
ALL SALES ARE FINAL once a requisition has been authorized.
Refunds are not granted for:
- Changing your mind
- Failure to visit the laboratory
- Disliking lab results
- Inability to complete fasting or preparation
- User error, including incorrect data entry
Refunds may be granted only if:
- We are unable to fulfill your order due to our error
- A technical issue prevents requisition delivery
- Services are not available in your state (determined after purchase)

7. Intellectual Property
7.1 Our Rights
All content, including text, graphics, logos, design, and software, is the property of 603 Health
and protected by copyright and trademark law.
You may not copy, modify, distribute, or use our content without written permission.
7.2 Your Rights
You retain ownership of information you provide. You grant us a limited license to use information
solely for delivering services, per our Privacy Policy.
8. Prohibited Uses
You agree NOT to:
- Submit false or misleading information
- Impersonate any person or entity
- Use SimpleLabs for illegal or harmful purposes
- Attempt to access restricted systems
- Use bots, scrapers, or automated tools without authorization
- Reverse engineer or interfere with our systems
- Resell or redistribute our services
- Use our services to diagnose, treat, or manage medical conditions
9. Third-Party Services
We rely on third-party partners for key service components:
- Fullscript / Rupa Health: Clinical authorization network
- Quest Diagnostics: Laboratory testing
- Stripe: Payment processing
Each partner operates under its own terms and privacy policies.
603 Health is not responsible for the actions, omissions, or policies of third parties.
10. Limitation of Liability
TO THE FULLEST EXTENT PERMITTED BY LAW:
603 Health is not liable for:
- Any indirect, incidental, special, consequential, or punitive damages
- Loss of profits, revenue, or data
- Medical outcomes based on lab results
- Decisions you make using information from our services
- Laboratory errors, delays, or service failures
- Actions of Fullscript / Rupa Health, Quest Diagnostics, or Stripe
Maximum liability for any claim shall not exceed the amount paid for your purchase.
Some jurisdictions may not allow certain limitations; only those permitted by law apply.
11. Indemnification
You agree to indemnify and hold harmless 603 Health and its partners from any claims arising from:
- Your use or misuse of services
- Your reliance on lab results
- Your violation of these Terms
- Your violation of another party's rights
12. Service Disclaimers
12.1 Service Availability
Services are provided "AS IS" and "AS AVAILABLE."
We do not guarantee:
- Continuous service
- Error-free operation
- Uninterrupted access
12.2 Lab Result Accuracy
We do not guarantee the accuracy or completeness of laboratory results.
Lab outcomes depend on numerous factors outside our control.
12.3 No Medical Outcome Guarantee
We make no representations about health improvements, diagnoses, or outcomes.
13. Changes to Terms
We may update these Terms at any time.
Updates are effective immediately upon posting.
Material changes will be communicated via:
Continued use after changes constitutes acceptance.
14. Termination

You may stop using our services at any time.
This does not entitle you to a refund.
14.2 By Us
We may suspend or terminate your access if you:

17. Entire Agreement
These Terms, together with our Privacy Policy, constitute the entire agreement between you and 603 Health.
